Arab telivér

A ló (Equus caballus) ma háziasított, vagy háziasításból visszavadult formában ismert. A különböző feladatokra különböző fajokat tenyésztettek, amelyek vérmérsékletük alapján lehetnek hidegvérűek, melegvérűek, nemesvérűek, pónilovak. Napjainkban munkához, sporthoz és szabadidős tevékenységekhez is használják őket. Gyógyszerészeti szempontból az ösztrogéntartalmú, vemhes kanca vizeletének (pregnant mares urine) van jelentősége, ugyanis a hormonpótló terápiás gyógyszerek túlnyomó többségének ez kulcsfontosságú összetevője. Az ebből készült gyógyszereket hormonpótló kezelésben vagy csontritkulás elleni terápiában alkalmazzák.

Az antibiotikumok használata jelentősen csökkentette a bakteriális fertőzésekkel összefüggő megbetegedést és halálozást. Ugyanakkor ezeknek a szereknek a helytelen, túlzott és sokszor felelőtlen használata az antibiotikum-rezisztencia probléma súlyosbodásához vezetett. A közösségben szerzett pneumonia (community acquired pneumonia, CAP) az egyik leggyakoribb akut fertőzés, és az empirikus antibiotikum-terápiája az antibiotikumok helytelen használatának a melegágyát képezi. A hazai és nemzetközi tanulmányok egyaránt azt mutatják, hogy a CAP-irányelvek betartása egyáltalán nem megfelelő a hatóanyag-választás és adagolás tekintetében, legyen szó otthon vagy kórházban kezelt betegekről. Az alacsony irányelv-adherencia alátámasztja azt, hogy nagyon nagy szükség van a helyi irányelvek hatékonyabb ismertetésére, könnyebb elérhetősége, felhasználóbarát volta pedig optimalizálhatja az antibiotikum-alkalmazást. Minden gyógyszert felíró orvos és gyógyszert kiadó gyógyszerész feladata, hogy antibiotikum-„őrzővé” (guardian) váljon, biztosítva, hogy antibiotikum csak valóban bakteriális fertőzés esetén kerüljön alkalmazásra, helyes szerválasztásban, megfelelő időben, adagolási formában és dózisban, illetve megfelelő ideig, optimalizálva így az antibiotikum alkalmazását már betegszinten is.

Empirical antibiotic therapy in community-acquired pneumonia (CAP)

The use of antibiotics has significantly reduced the bacterial infections-related morbidity and mortality. At the same time, the incorrect, excessive and often irresponsible use of these drugs has led to the escalation of the antibiotic resistance problem. As community-acquired pneumonia (CAP) is one of the most common acute infections, it might be a hotbed for antibiotic misuse. Both national and international studies show that CAP guideline adherence is not adequate at all in terms of antibiotic choice and dosage, whether it assessed in outpatients or inpatients. Poor guideline adherence highlights the need of more easily available and user friendly local guidelines that may improve antibiotic use. It is the duty of every prescribing doctor and dispensing pharmacist to become an antibiotic guardian, ensuring that antibiotics are used only in case of bacterial infections requiring treatment, with appropriate timing, dosage and duration, thus optimizing the use of antibiotics already at patient level.

A Janus-kináz- (JAK-) gátló gyógyszercsoport megjelenésével fontos taggal bővült a gyulladásos reumatológiai betegségek, mint például a rheumatoid arthritis vagy az arthritis psoriatica kezelésében felhasználható gyógyszerek palettája. Jelen összefoglalóban a tofacitinib, baricitinib, upadacitinib és filgotinib hatóanyagokra koncentrálva vesszük sorra azok farmakológiai tulajdonságait és a gyulladásos reumatológiai betegségekre vonatkozó felhasználási lehetőségüket, hatékonyságukat és biztonságosságukat.

JAK inhibitors in rheumatology

The therapeutic options in the treatment of the inflammatory rheumatic diseases (such as the rheumatoid arthritis or psoriatic arthritis) had expanded significantly in the recent years with the appeareance of Janus-kinase inhibitor group. In this summary we take a look at the pharmacological properties, efficacy, safety and mode of utilization of tofacitinib, baricitinib, upadacitinib and filgotinib in inflammatory rheumatic diseases.

BrAIN pályázat

A transzplantáció mára a végstádiumú szervelégtelenség egyik leghatékonyabb terápiája, azonban a posztoperatív időszakban felmerülő komplikációk továbbra is kihívást jelentenek a klinikus szakemberek számára. Az immunszuppresszív terápia kulcselemét képező kalcineurininhibitor hatóanyagokat szűk terápiás index jellemzi, az alacsony vérkoncentráció a beültetett szerv rejekciójához, míg a túldozírozás okozta magas vérszint súlyos mellékhatásokhoz vezethet. A kalcineurininhibitor terápia magas individuális variabilitása részben a citokróm P450 enzimek (CYP450) genetikai polimorfizmusára és a nem genetikai tényezők miatt kialakuló eltérő CYP3A aktivitásra vezethető vissza. Transzplantált betegek bevonásával végzett kutatásunk során sikerült összefüggést kimutatnunk a CYP3A-státusz és a takrolimuszvérszint (C0/D), illetve a terápiás hatáshoz szükséges dózis között. Jelenleg szívtranszplantáción átesett betegeknél vizsgáljuk az immunszuppresszív terápia következtében megváltozó, az NFAT (aktivált T-sejt nukleáris faktor) által szabályozott citokinek mRNS-expresszióját. Célunk egy új diagnosztikai rendszer kidolgozása, amely információval szolgálhat az immunszuppresszió hatékonyságának megállapításához, valamint segítséget nyújthat az esetleges kilökődési reakciók gyors diagnózisában a biopsziás mintából történő szövettani vizsgálatok kiegészítéseként.

Personalized immunosuppressive therapy

Transplantation is one of the most effective therapies for end-stage organ failure; however, it is still a challenge for clinicians due to the complications in the postoperative period. Calcineurin inhibitors, the key components of immunosuppressive therapy, act as modulators of the Nuclear Factor of Activated T-cells (NFAT)-regulated signalling by inhibiting the expression of cytokines responsible for graft rejection. These agents display a narrow therapeutic index, low calcineurin inhibitor concentrations can result in graft rejection, while high blood levels caused by overdosing can lead to serious side effects. The high interindividual variability of calcineurin inhibitor therapy is related to genetic polymorphisms of cytochrome P450 (CYP 450) enzymes and to the variations in CYP3A activities due to non-genetic factors. CYPtesting of transplant patients’ drug-metabolizing capacity revealed a correlation between CYP3A-status and blood levels of tacrolimus (C0/D) and the dose required for therapeutic effect. In collaboration with the Heart and Vascular Centre of Semmelweis University, the mRNA expression of NFAT-regulated cytokines altered by immunosuppressive therapy is investigated in peripheral blood samples from patients who underwent heart transplantation. Our aim is to identify new diagnostic biomarkers that can provide information about the efficacy of immunosuppression, can improve the rapid diagnosis of rejection reactions in combination with histopathology and allows monitoring of antirejection therapy.

Dr. Gulyás Eszter (képünkön) a pandémia idején írta dolgozatát, amelyben bemutatja, hogy a SARS-CoV-2-járvány hogyan formálta át a gyógyszerészetet, képet alkotva egy társadalmi-egészségügyi helyzetről, amely nemcsak a gyógyszerészek, de egy egész világ emlékezetében nyomot hagyott.

Praxis

A cink szervezetünk egyik elengedhetetlen nyomeleme, melynek hiánya számos kellemetlen tünet megjelenéséért lehet felelős. A lakosság jelentős része szenvedhet a cink hiányától, anélkül, hogy erről tudomása lenne. A cink kiemelt fontossággal bír az immunrendszer megfelelő működésében, így hiánya gyakran vezet különböző megbetegedésekhez. Emellett a cukorbetegségben is kulcsfontosságú a megfelelő cinkbevitel, hiszen számos folyamatban részt vesz és szabályozó szerepet is betölt. A sebgyógyulás különböző szakaszaiban is elengedhetetlen fontossággal bír, emellett a férfiak meddőségének is oka lehet a háttérben álló cinkhiány.

The the consequences of zinc deficiency, the importance and potential of zinc supplementation

Zinc is an essential trace mineral in human body, and its deficiency can be responsible for many uncomfortable symptoms. A significant proportion of the population may be suffering from zinc deficiency without even being aware of it. Zinc is essential for the proper functioning of the immune system, and its deficiency often leads to various diseases. In addition, adequate zinc intake is also crucial in diabetes, as it is involved in many processes and plays a regulatory role. It is also essential in the various stages of wound healing and can also be a cause of male infertility due to underlying zinc deficiency.

A glaukóma – közismert néven zöldhályog –összetett kórkép, mely a látóidegek jellegzetes és fokozatosan romló megbetegedésének tudható be. Kialakulásának elsődleges oka a látóidegfő és a retinalis idegrostréteg szabályozási zavara. A betegség általában együtt jár a csarnokvíz normális áramlásának megszűnésével, melynek hatására a szem belső nyomásának akut vagy krónikus megemelkedése jön létre. Az elnevezés mind a tüneteket, mind pedig a kiváltó okokat tekintve több igen eltérő betegség gyűjtőneve. Két fő megjelenési formája a nyitott zugú, illetve akut vagy krónikus zárt zugú zöldhályog. A glaukóma kezelhető betegség, aminek kiemelkedően fontos a korai felismerése, ugyanis a már kialakult károsodás nem visszafordítható, nem gyógyítható. A korai felismerés és az időben elindított terápia megelőzheti a látásromlás fokozódását és a glaukóma eredetű vakság kialakulását.

Glaucoma —The “silent thief” of sight

Glaucoma is a complex disease that can be attributed to a characteristic and gradually deteriorating disease of the optic nerves. The primary cause of its formation is the dysregulation of the optic nerve and the retinal nerve fiber layer, which causes chronic optic neuropathy. The disease is usually accompanied by the obstruction of the normal flow of aqueous humor, which results in an acute or chronic increase of intraocular pressure. The name is a collective name for se­veral different diseases in terms of both the symptoms and the underlying causes. Its two main types are the open-angle glaucoma and the acute or chronic closed-angle glaucoma. Glaucoma often requires a lifelong treatment. Its early detection is extremely important, because the already developed damage cannot be reversed or cured. Early recognition and therapy can prevent the visual impairment and the development of glaucoma-related blindness.
