A cirkadián ritmus az élő szervezetek belső, mintegy 24 órás periódusú biológiai órája, amely a szuprakiazmatikus mag vezérlésével szabályozza az alvás-ébrenlét ciklust, a hormonszekréciót, a testhőmérsékletet és számos élettani folyamatot. Ez a ritmus az életkor előrehaladtával jelentősen módosul: újszülöttkorban még kialakulatlan, majd fokozatosan érik, időskorban pedig amplitúdócsökkenés, fázis-előretolódás és fragmentálódás jellemzi, ami alvászavarokhoz és metabolikus eltérésekhez vezethet. E ritmicitás ismerete alapozza meg a kronofarmakológiát, amely a gyógyszerhatás napszaki és életkor szerinti változékonyságát vizsgálja. A ható­anyagok felszívódása, metabolizmusa és célszervi hatása napszaktól függően eltérő lehet, ezért az időzített gyógyszeradagolás (kronoterápia) növelheti a hatékonyságot és csökkentheti a mellékhatásokat. A közlemény összefoglalja a cirkadián szabályozás alapjait, az életkorral járó változásokat, valamint ezek klinikai és terápiás jelentőségét a személyre szabott gyógyszerelés szempontjából.

Age-dependent changes in circadian rhythm and chronopharmacology

The circadian rhythm is an organism’s intrinsic, approximately 24-hour biological clock, governed by the suprachiasmatic nucleus, which regulates the sleep-wake cycle, hormone secretion, body temperature, and numerous other physiological processes. This rhythm changes substantially with age: it is not yet established at birth, matures gradually during childhood, and in old age is characterized by reduced amplitude, phase advance, and fragmentation, which can lead to sleep disturbances and metabolic disorders. Understanding this rhythmicity underlies chronopharmacology, the study of how drug effects vary with time of day and age. The absorption, metabolism, and target-organ effects of active substances can differ depending on the time of administration, so timed drug dosing (chronotherapy) may increase efficacy and reduce side effects. This paper summarizes the molecular basis of circadian regulation, age-related changes therein, and their clinical and therapeutic relevance for personalized pharmacotherapy.

BrAIN pályázat

Munkánk során a patikai robottechnológia hatását vizsgáltuk a gyógyszertári dolgozókra és a betegellátásra vonatkozóan. Célunk volt megbizonyosodni arról, hogy a patikai robot­technológia gazdaságosan bevezethető a mindennapi gyakorlatba annak érdekében, hogy megkönnyítse a gyógy­szertár dolgozóinak munkáját és elősegítse a hasznos kommunikáció megvalósulását a szakemberek és a betegek között. Az adatokat kérdőívek segítségével gyűjtöttük össze. A kapott válaszok segítségével felderítettük a leggyakrabban előforduló hibákat, a robotokhoz kapcsolódó költségeket és a személyes véleményeket az új technológiával kapcsolatban. Megállapítottuk, hogy az expediáló robotok száma évről évre nő, az új technológia megkönnyíti a gyógyszerészek és az asszisztensek munkáját, valamint hozzájárulhat a mindennapi stressz csökkentéséhez. A robot használatának elsajátítása egyszerű, segítségével a lejárt gyógyszerek mennyisége és az expediálási hibák száma is csökkenthető. Eredményeink alapján a szakembereknek több idejük jut a betegek tájékoztatására, kérdéseik megválaszolására, ezáltal a betegellátás minősége jelentősen javulhat.

The impact of pharmacy dispensing robotics on pharmacy staff and patient care

The aim is for our study, to investigate the impact of pharmacy dispensing robotics on pharmacy staff and patient care. Ascertain whether pharmacy robotics could be cost-effectively integrated into everyday practice in order to facilitate the work of pharmacy staff and promote effective communication between healthcare professionals and patients. We collected the data using questionnaires. Using the responses received, we identified the most common errors, the costs associated with the robots, and personal opinions regarding the new technology. We found that the number of dispensing robots is increasing year on year; the new technology makes the work of pharmacists and assistants easier and can help reduce everyday stress. Learning to use the robot is straightforward, and it can help reduce both the quantity of expired medicines and the number of dispensing errors. Based on our findings, healthcare professionals have more time to inform patients and answer their questions, which could significantly improve the quality of patient care.

Praxis

A palmitoil-etanolamid (PEA) egy endogén zsírsav-etanolamid, amely gyulladáscsökkentő, neuroprotektív és fájdalomcsillapító hatásai révén az utóbbi években egyre nagyobb figyelmet kapott. Több támadásponton érvényesülő hatásmechanizmusa, elsősorban PPAR-α-aktivációja, a neuroinflammáció modulációja és az endokannabinoid rendszer indirekt szabályozása, amely hatások alapján számos gyulladásos és neuropathiás kórképben vizsgálják a vegyület terápiás potenciálját. A rendelkezésre álló klinikai bizonyítékok alapján a PEA kedvező hatásúnak bizonyult neuropathiás fájdalomban, diabeteses perifériás neuropathiában, osteoarthritisben, fibromyalgiában és fejfájásos kórképekben, miközben biztonságossági és tolerálhatósági profilja kedvező maradt. A mikronizált és ultramikronizált formulációk javított biohasznosulásuk révén tovább növelik a klinikai hatásosságot. A legújabb kutatások alapján a PEA alkalmazási lehetőségei a fájdalomkezelésen túl neurodegeneratív betegségekre, glaukómára, poszt-COVID neurológiai tünetekre és metabolikus eltérésekre is kiterjedhetnek. A jelenlegi evidencia alapján a PEA ígéretes kiegészítő terápiás lehetőség, azonban szélesebb körű klinikai alkalmazásának megalapozásához további jól megtervezett, nagy beteg­számú klinikai vizsgálatok szükségesek.

Palmitoil ethanolamide in the treatment of chronic pain – mechanisms of action, clinical evidence and new therapeutic perspectives

Palmitoylethanolamide (PEA) is an endogenous fatty acid ethanolamide that has gained increasing attention in recent years due to its anti-inflammatory, neuroprotective, and analgesic properties. Its multimodal mechanism of action, primarily involving the activation of PPAR-α, modulation of neuroinflammation, and indirect regulation of the endocannabinoid system, provides a rationale for its investigation in a wide range of inflammatory and neuropathic disorders. Current clinical evidence suggests that PEA may be beneficial in the management of neuropathic pain, diabetic peripheral neuropathy, osteoarthritis, fibromyalgia, and primary headache disorders, while demonstrating a favorable safety and tolerability profile. Micronized and ultramicronized formulations have shown improved bioavailability, which may further enhance clinical effectiveness. Emerging evidence also indicates potential therapeutic applications of PEA beyond pain management, including neurodegenerative diseases, glaucoma, post-COVID neurological manifestations, and metabolic disorders. Based on the available evidence, PEA represents a promising adjunctive therapeutic option, however further large-scale, well-designed clinical trials are required to establish ist optimal role in clinical practice.

A kerti kakukkfű (Thymus vulgaris L.), a 2026. év gyógynövénye, régóta alkalmazott gyógy- és fűszernövény, amelynek legfontosabb gyógyászati felhasználási területe a légúti panaszok, különösen a produktív köhögés kezelése. Cikkünkben áttekintjük a faj morfológiai jellemzőit, termesztését, feldolgozását, kémiai összetételét, hagyományos alkalmazását, valamint a preklinikai és klinikai vizsgálatok eredményeit. A Thymi herba legfontosabb hatóanyaga a főként timolt és karvakrolt tartalmazó illóolaj. A preklinikai vizsgálatok simaizom-relaxáns, szekretomotoros, gyulladáscsökkentő, antioxidáns és antimikrobiális hatásokat igazolnak, ezek klinikai jelentősége azonban csak korlátozottan tekinthető megalapozottnak. A humán vizsgálatok elsősorban a kakukkfüvet borostyánlevéllel vagy kankalingyökérrel kombináló készítmények produktív köhögéssel járó akut bronchitis tüneti kezelésének hatásosságát támasztják alá.

The Medicinal Plant of the Year 2026: Common thyme (Thymus vulgaris)

Common thyme (Thymus vulgaris L.), selected as the Medicinal Plant of the Year 2026 in Hungary, has a long history of use as both a medicinal and culinary herb. Its principal therapeutic application is the treatment of respiratory complaints, particularly productive cough. In this article, we review the morphological characteristics, cultivation, processing, chemical composition and traditional uses of the species, together with the findings of preclinical and clinical studies. The principal active constituent of *Thymi herba* is its essential oil, which contains mainly thymol and carvacrol. Preclinical studies indicate smooth muscle relaxant, secretomotor, anti-inflammatory, antioxidant and antimicrobial effects; however, the clinical relevance of these findings remains difficult to assess. Human studies primarily support the symptomatic efficacy of preparations combining thyme with ivy leaf or primrose root in acute bronchitis accompanied by productive cough.

Az Amerikai Reumatológusok Kollé­giuma (American College of Rheuma­tology, ACR) megújítja az axiális spondylo arthritis (axSpA) terápiás ajánlását. Az ajánlás legfontosabb megállapításait a honlapjukon már nyilvánosságra hozták és a megjelentetése is folyamatban van az Arthritis and Rheumatology következő számában. Annak ellenére, hogy új hatóanyag nem került be a terápiás fegyvertárba, néhány jelentősebb változás mégis lesz a terápiás megközelítésben.

Summary of treatment recommendations for relapsing-remitting axSpA (Bechterew’s disease)

The American College of Rheumatology (ACR) has updated its treatment recommendations for ankylosing spondylitis. The key findings of the guidelines have already been published on their website and are due to be published in the next issue of *Arthritis and Rheumatology*. Although no new active substances have been added to the therapeutic arsenal, there will nevertheless be some significant changes to the therapeutic approach.
