Animal bites: Practical aspects of initial management

Animal bites are common injuries whose initial management requires the simultaneous consideration of appropriate wound care, the risk of bacterial infection, the need for post-exposure prophylaxis against tetanus and rabies, and the indications for surgical assessment. Although management is influenced by the animal species, the type, depth, and anatomical location of the injury, the circumstances of exposure, as well as the patient’s immune status and previous vaccination history, the aim of this article is to provide a practical and unified overview of the initial management of animal bites, with particular emphasis on appropriate patient referral and the prevention of potentially serious complications.

The history of spinal muscular atrophy – New prospects for the treatment of neuromuscular disorders

5q-associated spinal muscular atrophy (SMA) is an autosomal recessive disorder caused by homozygous deletion or pathogenic variants of the SMN1 gene, in which selective degeneration of anterior horn alpha motor neurons leads to progressive proximal muscle weakness. This review traces the historical development from the entity described by Werdnig and Hoffmann to the identification of the SMN1 gene in 1995. In light of the molecular pathomechanism, the modifying effect of SMN2 copy number and the natural history of the disease, it evaluates the significance of nusinersen, onasemnogene abeparvovec and risdiplam. The author assess the prognostic impact of newborn screening and presymptomatic treatment, the open questions regarding long-term efficacy, and the lessons applicable to other neuromuscular disorders.

BrAIN pályázat

Az elmúlt évtizedek legfontosabb radiogyógyszerészeti fejlődései, a teranosztikus készítmények Magyarországon is elérhetővé váltak az utóbbi években. A teranosztika lényege, hogy ugyanaz a hordozómolekula szolgál diagnosztikára és terápiára; a jelző izotóp eltérhet. A neuroendokrin tumorsejtek felszínén expresszálódó szomatosztatinreceptorok célpontot adnak a receptorszelektív ligandumoknak, ezért ugyanazon szomatosztatinanalóg alkalmazható képalkotásra és peptid receptor radionuklid terápiára. A Lutathera nevű készítmény – lutécium-177 izotóppal jelölt oxodotreotid molekula – bevezetése áttörést jelentett, mert receptorpozitív neuroendokrin tumorokban célzott sugárzással képes csökkenteni a progressziót. A teranosztikus szemlélet prosztatadaganatos betegek körében is elterjedt. A prosztataspecifikus membrán antigént (PSMA) célzó radioligandok javítják a PET/CT‑alapú stádiummeghatározást, míg a PSMA-radioligand terápia előrehaladott esetekben szignifikánsan javította a túlélést. A Pluvicto nevű, 177Lu izotóppal jelölt PSMA‑ligandum ennek klinikai példája.

Theranostic innovations: Advances of radiopharmaceuticals in nuclear medicine

The most significant developments in radiopharmacy over recent decades – namely, theranostic agents – have also become available in Hungary in recent years. The essence of theranostics is that the same carrier molecule serves both diagnostic and therapeutic purposes; the tracer isotope may vary. Somatostatin receptors expressed on the surface of neuroendocrine tumour cells provide a target for receptor-selective ligands; therefore, the same somatostatin analogue can be used for imaging and for peptide receptor radionuclide therapy. The introduction of the preparation known as Lutathera – an oxodotreotide molecule labelled with the lutetium-177 isotope – represented a breakthrough, as it is capable of slowing progression in receptor-positive neuroendocrine tumours through targeted radiation. The theranostic approach has also become widespread amongst patients with prostate cancer. Radioligands targeting prostate-specific membrane antigen (PSMA) improve PET/CT-based staging, whilst PSMA radioligand therapy has significantly improved survival in advanced cases. Pluvicto, a PSMA ligand labelled with the 177Lu isotope, is a clinical example of this. Numerous clinical trials are currently evaluating new radiopharmaceuticals, including alpha-emitters, which may offer new indications and greater therapeutic efficacy.

Reflexió

Révné dr. Sugár Ágnes már aranydiplomás, patikai gyógyszerész kérdése:

Olvastam Budai Marianna cikkét a székrekedés kezeléséről.

Macrogol in pharmacy

Does molar mass matter?

The pharmaceutical and therapeutical application of Macrogols – or polyethylene glycols – is fundamentally determined by their molar mass. Macrogols with a molar mass exceeding 3000 g/mol are virtually unabsorbed from the gastrointestinal tract and are excreted unchanged in the feces. Their efficacy and safety in treating constipation are established. The question arises as to whether this also applies to Macrogols with lower molar masses.

Beszélgetősarok

Hosszú, munkával teli évtizedek állnak mögötte, amelyek során szakmai tudásával és elhivatottságával meghatározó szerepet töltött be a magyar gyógyszeriparban. Beszélgetőpartnerünk, dr. Péter Imre gyógyszerészként végzett a Semmelweis Egyetem Gyógyszerésztudományi Karán, 1984-ben. A Richter Gedeon Gyógyszergyárban kezdte pályáját, ahol 1999-től a Richter minőségirányítási igazgatójaként dolgozott, biztosítva, hogy a vállalat termékei a legszigorúbb minőségi és biztonsági előírásoknak is megfeleljenek – olyan felelősség ez, amely közvetlenül érinti emberek egészségét és életét.

Gyógyszerészet-történet

A BCR:ABL molekuláris drivernek a felfedezése megalapozta a tirozinkináz-gátlók (TKI-k) kifejlesztését. A CML kezelésénél végleg leszámoltak a nem specifikus gyógyszerekkel. Helyüket a célzott terápiás hatóanyagokkal végzett, a precíziós onkológia új paradigmája vette át.

25 years of imatinib, the first targeted therapy

The discovery of the BCR:ABL molecular driver laid the foundations for the development of tyrosine kinase inhibitors (TKIs). Non-specific drugs have been definitively phased out in the treatment of CML. They have been replaced by a new paradigm in precision oncology, based on targeted therapeutic agents. Imatinib mesylate (STI571) was first registered in 2001 under the trade name Glivec® (Gleevec® in the USA) for the treatment of chronic myeloid leukaemia (Chronic myeloid leukaemia, CML), thereby becoming the prototype of molecularly targeted, small-molecule anticancer therapy. By selectively inhibiting the constitutively active BCR-ABL1 tyrosine kinase, imatinib transformed a previously fatal malignant disease into a chronic, manageable condition: according to the long-term follow-up of the IRIS trial, the 10-year overall survival rate was approximately 83% [1, 2]. Its success insipred the development of second- and third-generation BCR:ABL1 inhibitors and validated kinase inhibition as a widely applicable therapeutic strategy, which was subsequently extended to gastrointestinal stromal tumours (GIST) and other KIT/PDGFRA-driven diseases [3]. This article reviews the pharmacological basis of imatinib, its clinical development, long-term outcomes, mechanisms of resistance, expanded indications, and the broader legacy of its quarter-century of use.
